1. Click this link DiSC Personal Profile Order Form (MS Word format) to download the form. Complete the form according to the instructions, attach it to your email and email it to disc@leetraining.com. If you use the Adobe Acrobat version, complete the form and fax it to (405) 356-4865
2. Once the order form is received, the online access and instructions will be emailed to the names you have specified.
3. As soon as the reports are completed, you will have immediate access to view, print, save or email them, as the need dictates.
